  Wedding Contract - To reserve Oak Haven Photography for your wedding, please print and fill out the Wedding Contract and return it with your deposit of 1/3 of your total payment.  When your deposit is received, your date will be reserved.  Until your deposit has been received, Oak Haven has the right to make other appointments for that day.

Wedding Information Sheet - Once your wedding date has been reserved, please print and fill out and return the Wedding Information Sheet.  It will help in planning the wedding day and avoiding any unnecessary conflict.

Wedding Checklist - Please print and fill out the Wedding Checklist to communicate what you would like photographed at your wedding.  What may seem obvious or standard to you may be different from what others are looking for.  Feel free to supplement this list with your own ideas or lists from other sources.  If you see images that you would like to try to recreate, feel free to mention it prior to your wedding day.  The more that is communicated prior to your wedding day, the smoother the day will go.

Order Forms
A La Carte Order Form - This form is for ordering one to several prints from the wedding.  To order prints, please print and fill out this form and send it in with your payment to Oak Haven Photography.

Summary Order Form - This form is used by the Bride & Groom to order their prints.  It is provided directly to the wedding couple.

Wedding Package Order Form - This form is used to order packages of prints, such as prints used to create a Bride's album.  It is provided directly to the wedding couple.
